First Initial Evaluation -

For patients under 18 years of age, the first initial evaluation will be with the parents/guardians of the patient.

For patients over 18 years of age there will be only one first initial evaluation.

  (45-50 min session) $495

Second Evaluation -

For patients under 18 years of age, the second initial evaluation will be for the patient.

  (45-50 min session ) $495

(45-50 min session) - $355

(25-30 min session) - $225

Extended patient telephone calls and calls to other professionals (attorneys, therapists, school personnel, etc.) will be charged at a rate of:

$140 per 15-min increment

Letters, reports, and forms will be charged based on the amount of time necessary to complete, and at the discretion of the provider.

Missed appointments and late cancellations are billed at the same rate as the scheduled length of time of your visit.
